Insurance and Billing
As a courtesy to our patients our office provides skilled insurance personnel to file claims to your insurance carrier for you. Your insurance will determine whether services are covered. You are ultimately responsible for any costs not covered by your health plan.
Please inform us of changes in your name, address, phone number, employer, health plan, group or policy number to make insurance filing more efficient.
Co-payments are due at the time of service. Patients who have overdue accounts, are not local residents or are new patients without insurance will be expected to pay in full at the time of the visit. You will be billed for deductibles, coinsurance and any non-covered services after we have received notification from your health plan.
We accept Visa, MasterCard and Discover for your convenience.
